Joining Questionnaire

This questionnaire is sent to all who join the Ekbom Support Group but completing it is not mandatory.  There is a question about whether you give permission for this information to be used for research and only if you agree is the completed questionnaire processed for research purposes.  Your name does not appear on the form, only a reference number.  We do earnestly ask that you do not put your name and address on the questionnaire as this defeats the anonymity we offer those who complete it.  The completed questionnaire is processed on a confidential basis.

We do not detail the questionnaire here. You will be sent your own copy for (optional) completion when you join us.
